Missed a GLP-1 Dose? What the FDA Labels Say About Semaglutide and Tirzepatide

The exact missed-dose windows in the FDA-approved Wegovy and Zepbound labels (48 hours and 4 days), what the labels say about skipping several doses, and why compounded products need a conversation with your prescriber.

A missed injection is one of the most common dose-week moments, and one of the few where the FDA-approved labels give specific, checkable numbers. The two big brand labels do not use the same window, which is why a rule from a forum thread or a friend on a different drug can be wrong for you.

Wegovy: the 48-hour rule

Section 2.4 of the Wegovy prescribing information, “Recommendations Regarding Missed Dose,” draws the line at the next scheduled dose. If one dose is missed and the next scheduled dose is more than 2 days (48 hours) away, administer WEGOVY as soon as possible. If the next scheduled dose is less than 2 days away, do not administer the missed dose; resume on the regularly scheduled day.

Zepbound: the 4-day rule

Zepbound’s label, Section 2.3, counts from the missed dose instead. A missed dose should be administered as soon as possible within 4 days (96 hours) after it was due. After more than 4 days, skip it and take the next dose on the regular day. The Medication Guide adds a floor: do not take 2 doses of Zepbound within 3 days (72 hours) of each other.

Missing more than one

For Wegovy, Section 2.4 says that after 2 or more consecutive missed doses, dosing can resume as scheduled or, if needed, the drug can be reinitiated on the dose escalation schedule, which “may reduce the occurrence of gastrointestinal symptoms associated with reinitiation of treatment.” That ties back to side effects: if your gap was long, nausea can return when you restart. Our nausea and vomiting FAQ covers what the labels say about when that stops being routine. The Medication Guide tells patients who miss doses for more than 2 weeks to resume on the regular day or call their provider about how to restart. We found no equivalent multi-dose instruction in the portions of the Zepbound label we read, so ask your prescriber.

Changing your injection day

Both labels permit it. Wegovy: as long as the last dose was given 2 or more days earlier. Zepbound: as long as at least 3 days (72 hours) pass between doses.

Compounded products are different

These windows belong to the approved brand products. A compounded vial has no FDA label, so any missed-dose rule comes from your prescriber and pharmacy, not the FDA. That is a practical reason to know which kind of product you are on and to ask for the instructions in writing. Our red-flags guide lists what the FDA says to check.
